I understand that judo is a contact sport involving throwing, grappling and falling, and that participation carries an inherent risk of injury including, without limitation, sprains, strains, fractures, concussion, serious injury or death.
I voluntarily assume all such risks, whether known or unknown, and confirm that the participant is physically fit to take part. I agree to follow all dojo rules and the instructions of the coaches at all times.
In consideration of being permitted to participate, I release and hold harmless kutō ie judo club, its coaches, volunteers, directors and facility owners from any and all claims, demands or causes of action arising from participation in club activities, except in cases of gross negligence.
I consent to emergency medical treatment if required, and I accept responsibility for any costs of such treatment. I understand membership fees of $65 per month per child (family deals: 2 kids = $50/month, 3 kids = $45/month) are due on the first of each month following the free trial week, and that fees are non-refundable.
I grant permission for photographs or video taken during club activities to be used on the club website and social media. (Tell a coach in writing if you wish to opt out.)
If the participant is under 18 years of age, I confirm that I am their parent or legal guardian and I sign this waiver on their behalf.
